Tear Trough Filler – A Real Client Journey to Brighter, Fresher Eyes

When my client first came in for a consultation, she wasn’t sure whether tear trough filler was right for her. She had been struggling with dark under-eye shadows and what appeared to be an under-eye “bag.” Her biggest worry was a valid one: “Will filling this area actually make the eye bag worse?”


And honestly, that concern makes perfect clinical sense. You never want to directly fill an eye bag—adding gel to an area of existing heaviness can only make the puffiness and shadowing more noticeable. The success of any under-eye treatment starts with a precise examination, not simply injecting product.



The First Step: Proper Under-Eye Assessment


For any tear trough filler treatment—whether it's a true tear trough (a hollow, sunken appearance) or a false tear trough (shadowing caused by an eye bag)—the assessment determines everything.


Here’s what we look for:


*Is the under-eye suitable for filler?* Sometimes an alternative treatment is better.

*Skin elasticity and quality:* Will the filler sit smoothly, or will it migrate or pool?

*What is causing the shadow?* Hollowing, laxity, or an eye bag?

*Where should the filler be placed to help—if it can help at all?*


In this case, the client’s issue wasn’t hollowing, but an eye bag causing uneven light reflection and a dark shadow. The solution wasn’t to fill the bag—it was to place filler around the area to blend it seamlessly into the cheek and lateral region. This evens the surface, improves light distribution, and reduces the appearance of shadow.



A Real Example: How We Treated This Eye Bag


Although many people think eye bags can’t be improved with filler, they can—with careful placement, precision, and deep anatomical understanding.


For this client, the aim was to:


* Blend the bag into the surrounding tissue

* Reduce dark shadows

* Create a smoother, fresher under-eye

* Avoid heaviness or puffiness


We used Teoxane Redensity 2, one of the best tear trough fillers designed specifically for this delicate area. It has a lightweight, silky consistency that integrates naturally and avoids the risk of lumpiness.


The technique:


1. A tiny entry point is created using a fine needle—just a momentary pinch.

2. A cannula (a blunt, flexible tool) is then used to minimise bruising and reduce risk of complications. This is one of the reasons the treatment is considered so safe.

3. Strategic placement in the malar region and laterally, allowing us to blend the eye bag into the cheek contour rather than fill directly under the puff.


We used just 1 mL, distributed evenly—around 0.45 mL on one side and 0.55 mL on the other. Results were visible immediately, with full settling expected in 3–4 weeks.

* Temporary unevenness while the filler integrates


Because we use a cannula and a specialised under-eye product, more serious complications are extremely rare. We always offer check-ins at 1–2 weeks if needed.


How Long Do Tear Trough Fillers Last?


A question I hear almost every day is: How long do tear trough fillers last?


The honest answer is: it depends on you.

Everyone metabolises filler differently, but with Teoxane Redensity 2, longevity is typically excellent.


My clinical experience:


Most clients need *only 1 syringe** at the first treatment

* Some may need a touch-up in the initial 2-session plan

Although the product guideline says 6–9 months, *most clients return around 12 months—or even later**

* The second session usually requires less filler than the first


This makes it a strong long-term investment for rejuvenating the under-eyes.

This client’s photos were taken soon after treatment, so she may experience early minor unevenness due to initial swelling or settling. But by week four, the result will be even smoother, softer, and fully integrated.


Her under-eye shadows are reduced, the bag is blended, and her eyes look brighter and far less tired—a refreshed look without surgery.
